What information will NML collect about you in the survey?
If you do take part in this survey, NML will, of course, collect the information you provide when you complete the questionnaire. For some existing customers, additional information about your account could be appended to your survey responses. This could include the length of time your organization has been a customer.
The survey will also collect your IP address as noted previously, plus the time you take to complete the survey and your geographic location at the time of taking the survey.
How will NML use the information you provide in the survey?
All the information you provide will be stored separately from the personal data used to invite you to participate in this research. In fact, the invitation to participate was not handled by Nucleus Marketing Lab so NML did not have access to that personal data.
Your participation in this research, together with your individual responses to the questions posed in the survey, will be kept strictly confidential by NML.
Your responses to this survey will be used solely for research analysis purposes. The results of this research will be anonymous and we can assure you that you will NOT be identifiable in any published results. In most cases, information is reported at the aggregate level.
In addition, if you contact NML for any reason regarding this research, your contact details will be used solely for the purpose of responding to your request.
You information will not be passed on to any third party beyond NML’s use of the research data for the purposes already described, with the exception of email address data voluntarily provided by participants for the Incentive Opportunity.
If you have any questions about this research or how your information will be used, please contact the NML team. They can be contacted by email sent to firstname.lastname@example.org

In addition, subcontractors who may carry out some data processing or quality control on behalf of NML may also have access to the information. The individuals handling your information are based in North America.
Will this survey collect information from children?
No, this survey does not involve collecting information from children. For research involving children, our established practice is to obtain consent from a parent before inviting a child to take part in the research. However, as noted, we are only inviting adults to participate in this particular research survey.
How long will NML keep the information you provide?
Noting that no personal information was provided to NML for this research, the personal information that is voluntarily self-reported by research participants will be kept for no more than twelve months from the completion of the study.
The anonymous survey results will be kept without any defined date for destruction as no personal data is involved.
We also automatically capture information about your operating system, display settings and browser type in order to ensure that the survey questionnaire is delivered in a form suited to the software your computer is using. We do not capture any other information from your computer.
How does NML ensure your personal data is held securely?
We take precautions to ensure your information is protected from loss, theft, or misuse. These precautions include physical security of our office location, controlled access to computers, and use of secure, encrypted internet connections when collecting personal data.
How can I correct the information held about me, request that my information is destroyed or handle other disputes?
You have the right to request a copy of all the information we hold about you. Second, you have the right to request the correction of any personal data we hold about you.

What should I know about the incentive being offered for this study?
From time to time, NML offers an incentive or honorarium to qualified participants who complete a study. The incentive is offered on an “opt-in” basis, meaning you can complete the survey and choose not to be part of the incentive distribution. If you do elect to opt in, personal data is required for the fulfilment of the incentive. Please see more details regarding the Incentive Opportunity below.
No Purchase Necessary
No purchase is required for the incentive.
Entrants must be eighteen (18) years of age or older. Void where prohibited. The incentive is open only to individuals included in the original invitation for this survey. Employees of Nucleus Marketing Lab and its affiliates, as well as the immediate family (spouse, parents, siblings, and children) and household members of those employees, are not eligible to enter.
The incentive eligibility period begins as indicated on the invitation and within the survey (the “Entry Period”). Entries submitted before or after the Entry Period will not be eligible.
How to Enter
Complete the survey after receiving an invitation to participate in the survey. Complete the optional incentive information section, also known as the registration section, at the end of the survey. This registration denotes an elective opting into the possibility of receiving an incentive for completing the study (the “Incentive Opportunity.”)
There is a limit of one (1) entry per person and you must provide a valid email address to enter. No other methods of entry will be accepted. The use of an agency or any automated system to enter is strictly prohibited, and we reserve the right to disqualify any entries received through such methods, as determined by NML, in its sole discretion.
Receiving An Incentive
According to the timeframe indicated in the survey, NML will email the incentive recipients based on their order of completion of the survey. NML or its agent will attempt to notify potential recipients(s) via the email address provided within the timeframe noted in the survey. If a recipient cannot be contacted or is disqualified for any reason, NML reserves the right to determine an alternate recipient or not to award the recipient’s prize, in its sole discretion. Recipients will receive an email from email@example.com.
The recipients will be awarded the incentive on or before the date indicated in the survey. Prizes will be emailed to the recipients at the email address provided on the registration form. NML is not responsible for inaccurate email addresses or typographical errors in registering. This incentive is not sponsored by or affiliated with Amazon, Home Depot, Target, Walmart or any of the other brands noted as potential awards a recipient can select. A recipient is responsible for all expenses not specified above, including without limitation, paying all federal, state, local and applicable income taxes and any and all other costs and expenses not listed above. Any prize details not specified above will be determined by NML, in its sole discretion. All prizes are awarded as-is without any warranty or guaranty, either express or implied by NML. A prize may not be transferred and must be accepted as awarded. A recipient may not request cash or a substitute prize.
In the event that the operation, security, or administration of the incentive is impaired in any way, NML may, in its sole discretion, either: (a) suspend the Incentive Opportunity to address the impairment and then resume the Incentive Opportunity; or (b) award the prize(s) at random from among the eligible entries received up to the time of the impairment. NML reserves the right, in its sole discretion, to disqualify and seek damages from any individual who tampers with the operation of the incentive portion of the survey, violates these Official Rules, or acts in a disruptive or unsportsmanlike manner. NML’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision. Proof of sending any communication to NML by mail shall not be deemed proof of receipt of that communication. In the event of a dispute as to the owner of an online entry, the authorized account holder of the e-mail address used to enter will be deemed to be the owner. The research Incentive Opportunity is subject to federal, state, and local laws and regulations and is void where prohibited.
For all incentives, NML disclaims all liability for any delays, misdelivery, loss or failure in the delivery of any item sent by mail, courier, express, electronic transmission, or other delivery method.
Release and Limitations of Liability
By participating in the Incentive Opportunity, you agree to release and hold harmless NML, its respective parents, subsidiaries, affiliates, and each of their respective officers, directors, employees, and agents (the “Released Parties”) from and against any claim or cause of action arising out of your participation in the incentive opportunity or receipt or use of any prize, including, but not limited to: (a) unauthorized human intervention in the Incentive Opportunity; (b) technical errors; (c) printing errors; (d) lost, late, postage-due, misdirected, or undeliverable mail; (e) errors in the administration of the Incentive Opportunity or the processing of entries; (f) breach of any contractual or fiduciary relationships; or (g) injury or damage to persons or property. You further agree that in any cause of action, the Released Parties’ liability will be limited to the cost of entering and participating in the Incentive Opportunity, and in no event shall the Released Parties be liable for attorney’s fees. You waive the right to claim any damages whatsoever, including, but not limited to, punitive, consequential, direct, or indirect damages.
Privacy and Publicity
Except where prohibited, you agree that any and all disputes, claims and causes of action arising out of, or connected with, the Incentive Opportunity or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in Arizona. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, your rights and obligations, or the rights and obligations of NML in connection with the Incentive Opportunity, shall be governed by, and construed in accordance with, the laws of Arizona, without giving effect to any choice of law or conflict of law rules (whether of Arizona or any other jurisdiction, either domestically within the United States of America or globally), which would cause the application of the laws of any jurisdiction other than Arizona.
